What Is Social Anxiety Disorder?
Social anxiety disorder is an intense fear of social situations. If you have it, you might dread being judged, embarrassed, or looked down on by others. That fear can feel so overwhelming that you start avoiding situations most people don’t think twice about like speaking up in class, eating in public, or making a phone call.
This is not the same as being shy. Shyness is a personality trait. Social anxiety disorder is a clinical condition that gets in the way of your work, school, relationships, and daily life. The good news: it responds very well to treatment.
Social anxiety disorder is not a character flaw or a sign of weakness. It’s a well-understood condition and most people who get the right treatment see real, lasting improvement.
Signs and Symptoms of Social Anxiety
Social anxiety shows up in how you think, how you feel in your body, and what you do. Common signs include:
- Intense fear of being judged or embarrassed
- Avoiding parties, gatherings, or social events
- Fear of speaking up in class or meetings
- Anxiety about eating or drinking in front of others
- Dreading phone calls or video meetings
- Avoiding eye contact
- Replaying conversations and cringing afterward
- Blushing, sweating, trembling, or feeling sick
- Racing heart or trouble breathing in social settings
- Showing up late or leaving early to avoid people
- Drinking alcohol just to get through social situations
- Canceling plans because of anxiety
Social anxiety can be focused on one specific fear like public speaking or it can affect almost every social situation. Either way, treatment works.
Why Does Social Anxiety Keep Getting Worse?
The more you avoid social situations, the stronger the fear gets. That’s the trap. Every time you skip a party, cancel plans, or stay quiet in a meeting, your brain gets the message that the situation was truly dangerous. So next time, the anxiety is even louder.

Two things keep social anxiety going:
- Negative thoughts: You assume people are judging you harshly. You predict you'll say something embarrassing. You expect the worst. These thoughts feel like facts but they're not.
- Safety behaviors: These are the small things you do to feel safer over-preparing what to say, avoiding eye contact, staying glued to your phone at a party. They bring short-term relief, but they stop you from learning that you were actually okay.
- Avoidance: Skipping situations gives you instant relief. But it makes the anxiety bigger over time. Facing the fear step by step is what shrinks it.
How We Treat Social Anxiety
We use two treatments that have the strongest research support for social anxiety: Cognitive Behavioral Therapy (CBT) and Exposure and Response Prevention (ERP). Both are short-term, skills-based, and designed to create real change in your daily life.
Cognitive Behavioral Therapy (CBT)
CBT helps you spot the thoughts that are fueling your anxiety and test whether they’re actually true. For example: Do people really notice your blushing as much as you think? Are the consequences of an awkward moment really as bad as you fear? Most of the time, the answer is no. CBT also tackles the habit of replaying social situations afterward and only focusing on what went wrong.
Exposure and Response Prevention (ERP)
ERP is where the real change happens. It means gradually facing the situations you’ve been avoiding without using safety behaviors so you can see for yourself that you can handle it. We build a step-by-step plan together, starting with situations that feel manageable and working up from there.

For example, a typical plan might look like this:
- Starting out: making small talk with a cashier, asking a stranger for directions, or ordering at a counter
- Building up: joining a group conversation, attending an event solo, or making a phone call without a script
- Bigger steps: speaking up in a meeting, giving a presentation, or going to a party and starting conversations
Dropping safety behaviors is a key part of ERP. If you go to a social event but spend the whole time on your phone, the fear doesn’t budge. Your therapist will help you identify what you’re doing to stay “safe” and help you let go of it, one step at a time.

Research shows that CBT and ERP work just as well over video as they do in person. Virtual sessions can also be a helpful first step for people whose social anxiety makes it hard to come in.

What's the difference between social anxiety and shyness?
Shyness is a personality trait some people are just more reserved, and that’s okay. Social anxiety disorder is different. It’s a persistent, intense fear that gets in the way of your life. If anxiety about social situations is stopping you from doing things you want to do, that’s worth talking to someone about.
Can social anxiety go away?
Yes many people reach full recovery with CBT and ERP. Others get to a place where social situations are manageable rather than overwhelming. Either way, you don’t have to stay where you are. The research on these treatments is strong, and the results tend to last.
Do I need medication?
No medication isn’t required. CBT and ERP work well on their own and address the root cause of social anxiety, not just the symptoms. Some people do use medication alongside therapy, and that can be appropriate in certain cases. We can talk through what makes sense for you.
Do you treat kids and teens with social anxiety?
Yes. Social anxiety is very common in children and teenagers especially around school, friendships, and performance situations. We use age-appropriate CBT and ERP, and we work closely with parents to support progress between sessions.
Can I do this over video?
Absolutely. Virtual sessions are available throughout Florida and New York. Research confirms that CBT and ERP are just as effective via telehealth. Some clients actually find it easier to practice exposures in the real-world places that matter to them with their therapist guiding them over video.
Is social anxiety related to OCD?
They’re separate conditions, but they can overlap. Both involve intrusive thoughts and behaviors that temporarily reduce anxiety but keep it going long-term. We specialize in both, and we know how to tell them apart and treat them together when needed.
