Pure O OCD, often called “Purely Obsessional OCD,” is a form of obsessive-compulsive disorder (OCD) in which the compulsions are primarily mental rather than visible behaviors. Individuals with Pure O experience distressing intrusive thoughts and respond by engaging in internal mental rituals such as rumination, analyzing thoughts, or trying to figure out why the thought occurred.

Although the term “Pure O” suggests that there are no compulsions, this is not entirely accurate. People with this form of OCD do perform compulsions typically consisting of avoidance combined with mental compulsions rather than the other common physical behaviors.
